The uncinate process and ethmoid bulla form the entry point to functional endoscopic sinus surgery. Identifying these structures correctly is the first step in any safe middle meatal antrostomy, and confusion here is one of the most common sources of error for trainees. A clear mental map of their relationship to the orbit and skull base is essential before any instrument enters the nose.

The skull base and lamina papyracea as safety boundaries
Two structures define the limits of safe dissection in the ethmoid sinus — the skull base above and the lamina papyracea laterally. Recognising their position at every step of a dissection is what separates a controlled, confident procedure from one carrying real risk of orbital or intracranial injury.

The basal lamella and posterior ethmoid anatomy
As dissection progresses posteriorly, the basal lamella of the middle turbinate marks the boundary between anterior and posterior ethmoid cells. Understanding this landmark allows surgeons to navigate safely toward the sphenoid sinus without losing orientation.
